The simple process of checking and recording your blood pressure at least twice a month over a period of four months has been shown to lower blood pressure in some people with hypertension.
The Y’s Blood Pressure Self-Monitoring program is 4-month program that includes:
- 10-minute consultations with a Healthy Heart Ambassador during virtual office hours
- weekly check-ins from a Healthy Heart Ambassador by phone, email or text
- monthly nutrition education seminars.
Participants of the program are asked to attend two personalized consultations per month (office hours) and the monthly Nutrition Education Seminars.
