Blood Pressure Self Monitoring Program

The simple process of checking and recording your blood pressure at least twice a month over a period of four months has been shown to lower blood pressure in some people with hypertension.
The Y’s Blood Pressure Self-Monitoring program is 4-month program that includes:
  • 10-minute consultations with a Healthy Heart Ambassador during virtual office hours
  • weekly check-ins from a Healthy Heart Ambassador by phone, email or text
  • monthly virtual nutrition education seminars
Participants of the program are asked to attend two personalized consultations per month (office hours) and the monthly virtual Nutrition Education Seminars.
Thanks to the generosity and support of the Brookfield Education Foundation, we are enrolling 10 qualified Brookfield Residents to participate in the program funded by the Brookfield Education Foundation.
